Required Documents
  1. Application Form
  2. Copy of Company’s Article of Association and Ministry of Law & Human Rights Decree
  3. Copy of Amendment to the Company’s Article of Association with Ministry of Law & Human Rights Decree
  4. Copy of Nomor Induk Berusaha (NIB)
  5. Copy of Business License (SIUP) or other similar document
  6. Copy of Company’s Tax ID (NPWP)
  7. Copy of Domicile Letter or Similar letter
  8. Copy of Board of Directors and Board of Commissioners’ ID Card (KTP/Passport/KITAS)
  9. Copy of PIC’s ID Card (KTP)
  10. Copy of Cardholder’s ID Card (KTP/Passport, KITAS) and NPWP
  11. Audited Financial Statement of last 2 years (preferably audited by external auditor)
  12. Last 3 months Bank Statement
  13. Company Profile
Note:
  • Required documents for additional issuance card : Application Form, KTP/Passport & KITAS, and Personal Tax ID (NPWP).
  • To be the main Authorized Corporate Officer, the individual should be listed in the Article of Association as President Director; if not, a Power of Attorney from the person in the article of Association will suffice.
